Both the core text, Heart of Darkness by Joseph Conrad and its film appropriation, Apocalypse Now directed by Francis Ford Coppola, ignite interest as to question whether humans are essentially creatures of dark nature when stripped down to bare essentials. When these are linked to values of greed and hunger for power and domination, these forces emerge through extreme characterization. The metastases are in the lymph nodes of the left axilla, both the lungs and the liver. However, the primary lesion was removed by surgery three years ago but it was aggressively metastatic as it was related to BRACA1 gene. It was two months ago that she developed the axillary, hepatic and pulmonary metastases. She lives with Alice, who is her partner and two children, an 11-year-old girl known as Catherine and a 7-year old boy known as Geoffrey. At a multinational corporation, Philippa was working as a financial manager but ceased after the confirmation of the metastases. Following this, she had to resign from work. Philippas partner works from home as an interior designer but the condition of the partner is overwhelming demanding much time. She is the only person who can care for Philippa and the children. Philippa is experiencing symptoms and signs such as an itching skin, anorexia, abdominal swelling, fatigue, oral thrush, xerostomia, and shortness of breath on exertion. Alice helps in the management of these manifestations. Currently, Philippa is on medications as follows: Oral solution of Nystatin, 1ml after every six hours per day Metoclopramide 10mg after every 8 hours and has to be taken 30 minutes before having meals. 2 puffs of salbutamol inhaler four times per day. 10mg of prednisolone two times a day. Aim: The aim of this poster is to prioritise five nursing issues that arise from Philippas assessment and identify two most critical ones based on the severity as an effect to the patient and Maslows hierarchy of needs. Nursing issues: Ineffective breathing patterns Imbalanced nutrition, less than body requirements Risk for electrolyte and fluid imbalance Risk for impaired integrity of the skin Risk for complicated grieving (MacAvoy, Moritz, 2013) The two main nursing issues to be considered are; ineffective breathing patterns and imbalanced nutrition, less than body requirements. Ineffective Breathing Patterns. The metastases from the primary lesion of the left breast reached Philippas left and right lungs. As such, both of them have cancerous cells that impair the respiratory function of the lung parenchyma. This is confirmed by Philippas shortness of breath on exertion, cough and dry mouth, oral thrush and fatigue, which result from ineffective breathing ("Respiratory System", 2015). The three major nursing issues that must always be assessed first even during resuscitation are the airway, breathing and blood circulation because they are essential for delivery of basic nutrients to the bodys vital organs such as the liver, the kidneys, the heart and the brain. Because Philippa has shortness of breath, the amount of air delivered to the alveoli is lower and subsequently, the vital organs will receive less oxygen (Bockov, Mare?kov, Zapletalov, 2015). This can be identified by taking the saturation rates of oxygen in the blood. Additionally, the impaired pattern of breathing triggers a physiological response that forces Philippa to breathe through the mouth. When oral inhalation is prolonged, further xerostomia shall be witnessed and mouth sores develop. Additionally, Philippa has oral thrush due to the xerostomia and reduced functions of the lungs. Prednisolone is prescribed to prevent further inflammation of the mouth and other parts of the body (Tanser, 2017). T he shortness of breath also causes a straining effect on Philippas diaphragm and the intercostal muscles. Consequently, she developed the fatigue as witnessed on assessment. Because the Philippas lungs are neoplastic due to the metastases, there is need to prioritise and stabilise her breathing because. e any further delay would lead to respiratory arrest and eventual death. Again, this is the reason she is on salbutamol inhaler to dilate her airway (Smith, 2012). Additionally, nystatin is given to prevent the spread of metastatic cells (Sambo, Tumia, Bradaschia, Pavat, Szymska, 2016). Risk For Imbalanced Nutrition, Less Than Body Requirements. Philippa is at risk of lacking vital nutrients such as glucose, electrolytes, proteins, vitamins, water among others in the body. Various reasons underlie this fact. First off, she has reduced appetite, a dry mouth and a swollen abdomen. Lack of appetite results in the refusal to ingest food and therefore, the absorptive sites of the gastrointestinal tract are less active. The body will not receive energy that is required for cellular respiration and other activities. Furthermore, water is not also taken. Water is vital for blood formation, dilution of toxins and excretion of substances through urine and faeces. Essentially, every body organ needs water for all functions. Again, Philippa has a dry mouth, indicating that saliva is insufficient (Hogan, Gingrich, 2014) The saliva is necessary for lubrication of food for easy peristalsis and digestion. Therefore, a dry mouth will not facilitate swift digestion. One of the major reasons the patient has fatigue is due to the loss of appetite and subsequent deficiency of energy in the body. Philippas swollen abdomen is due to the cancerous cells in the liver. As such, there is impairment of the hepatic functions of detoxification, haematopoiesis, bile manufacturing, clotting factor-formation, protein synthesis, storage and release of glycogen, production of immune factors, clearance of bilirubin among others (Patel, Abate, 2013). Bile salts play a vital role in emulsification and because Philippas liver has cancerous cells, this role is deterred in the duodenum (Heatley, 2013) . The swollen abdomen also may cause pain after eating because there is pressure in the hepatic portal vein that takes nutrient-rich blood to the liver resulting from hepatomegaly (Heatley, 2013) Nutrients play important roles in the body and should always be available to the body. For instance, the brain does not utilise any other form of sugars apart from glucose. If glucose is not available, then cerebral functions are impaired and may eventually cause coma (Lippincott, 2013) Moreover, these nutrients are needed to create energy that is used during movement, basal metabolic rate, cell division, immunity, cognitive functioning, increase in body size among others. Therefore, if they are not consumed, then all the above functions are affected and may lead to eventual death. Therefore, necessary and effective remedies need to be established to help Philippa regain the nutrients needed for her body to function effectively. For instance, a large-bow cannula can be introduced to administer Ringers lactate, glucose, and normal saline to the blood (Lippincott, 2013) Again, a nasogastric tube may be introduced to administer food supplements directly to the stomach. Conclusion Philippa was diagnosed with metastatic breast cancer that spread to the lungs, axillary lymph nodes and the liver. Additionally, she is currently on medications that are sustaining her. The major nursing issues identified in her case includes ineffective patterns of breathing, imbalanced nutrition, less than body requirements, risk for fluid and electrolyte imbalance, risk for complicated grieving and risk for impaired skin integrity. Much focus was on the patterns of breathing and imbalanced nutrition. Breathing is important for delivery of oxygen to the tissues. On the other hand, nutrients are vital for creation of energy, enhancing metabolic functions, body building, strengthening the immunity and other functions. This crude form of surgery was proven to be obsolescent when Ambrose Pare, a surgeon at the siege of Turin, discovered a more efficient and less painful way to dress wounds. He used a mixture of egg yolk, rose oil, and turpentine. This method treated patients with less pain, no swelling, and less inflammation than ones treated with boiling oil. Before his time, Renaissance doctors continued to follow the methods of the ancient Greeks. They relied mostly on the works and discoveries of Galen, a Greek physician of the 2nd Century AD. He ba sed most of his conclusions about human anatomy from dissections on Barbara Apes. As a student attending the University of Pad, Versatile made his own ideas of the human anatomy. But the discovery did not stop there. In 1718, a French surgeon, J. L. Petit, found a way to control the flow of blood during thigh amputations by inventing an effective tourniquet. (Dunn et al. 40) This was one of the most useful surgical tools used in the Renaissance time because of its effectiveness. It made leg amputations, which were common, much less time consuming. This was a blessing for the patients, considering there were no anesthetics. Worldwide slavery is in the millions† But the issue doesn’t stop at just slavery but at also includes sex trafficking and prostitution. (p. 506) In Kate Butchers article she discusses the difference between prostitution and sex trafficking and that solution to this problem is to address and promote human rights and working with laws that are already in place to address health issues such as HIV/AIDS and the human rights of people in the sex industry. Millers article he believes that sex trafficking should be put under strict regulations and do to so we must support and ask for action to be done at home and aboard and the government must have willingness to impose economic penalties on counties that give antislavery laws meaning. His method is to be â€Å"all so slow and so controlled† (217) at first, to lure the bull into proximity.Similarly, Brett lures men in with her brazen sexuality. The end result is the same however, with the bull and the men she's involved with. Romero will fight and kill the bull, just like Brett will lure men and eventually leave them. It's â€Å"all for sport, all for pleasure† (208). Brett's attraction to Romero can be explained in terms of his masculinity. Romero is somewhat of a perfect male, he's an aficionado, brave, beautiful, and held in very high esteem by everyone he encounters as well as being the favorite fighter of the crowd. He resembles Brett's masculinity.S he's given a unnisex name, has short hair, and even refers to everyone as â€Å"chaps. † She seems to mingle only with other men, possibly because she's shunned by other females for her promiscuity. While Romero and the bull most closely represent Brett and Jake, upon closer inspection another similarity can be drawn between Belmonte and Robert Cohn. Belmonte is a symbol for Cohn and his relationship with the other characters. Just like how Belmonte serves as a foil to the decadent style of Mercial and valor of Romero, Cohn is a foil to the other characters around him.While Romero would tease the audience with his graceful style and fighting close to the bull, Belmonte, now resuming bullfighting out of retirement, simply â€Å"gave the sensation of coming tragedy† and only gave the illusion of being close to the bull. He was once a great fighter, but now refuses to subscribe to the new â€Å"decadent† (215) bullfighting style. His fighting style represents Cohn in the sense that they both had their shining moment—Cohn's brief relationship with Brett, and Belmonte's glory days of bullfighting: which they're still trying to win back.Belmonte stands apart from the other fighters because he adheres to an traditional form of fighting. Similarly, Cohn is the only character with no involvement in the war, he preserves a sort of innocence and value system that disappeared with the â€Å"lost generation. † He tries to preserve the idea that sex equals intimacy and love, and throughout the novel his clumsy attempts to win back Brett are reflective of his refusal to understand that his brief relationship with Brett was simply a series of sexual encounters, nothing more.In conclusion, part of what makes Hemingway's style so unique is that he simply shows, without much telling. The matador scene in chapter 18 is rich because it provides penetrating insight with it's symbolism; Lady Brett with her elusive nature with men is captured throug h Romero's matador technique. The fact that Romero penetrates the bull with his sword accentuates the inherent masculinity that Brett displays—a sort of role reversal. Chapter 18 also highlights the character of Cohn, and his role as a foil and it's parallels to Belmonte's traditional fighting style.

Memento (2001), a film that was written and directed by Christopher Nolan, has captivated the attention of movie and puzzle enthusiasts everywhere. Some may call it a Drama or Mystery, or even a Thriller or a Neo-Noir film, whatever the case is, this film fits most, if not all, of these categories. Memento is a film about a man, Leonard Shelby who lives his life with Anterograde Amnesia which leaves him incapable of making new memories. He spends the time in this film putting pieces of a puzzle together that will hopefully lead him to find the man who raped and killed his wife. Christopher Nolan takes us through a crazy journey that involves twists and turns, Fear and Anxiety and even a feeling of Paranoia that leaves us questioning who we should really trust in the film. This psychological mystery allows us to see the film as neo-noir because of the flawed protagonist which is reflected in the main character, Leonard. With the disability our main character and narrator has, it might give some viewers a feeling of uneasiness and may even not trust him entirely which gives the feeling of paranoia in not knowing who in the film is telling us the truth and who is lying to us. The story is told in a confusing way, if one were to not pay attention for a short period of time, it may be certain that you will be lost in the film. One has to truly pay up most attention in order to fully process everything that is going on, one viewing may not even be enough, a second viewing may help us catch things that were previously missed or even hidden in plain sight, but the film keeps us with the anxiety of wanting to know what happens next which allows this film to catch our attention even after one viewing. Memento is a film that can fall under many genres of film, I will however, be focusing on the genres of Drama, Mystery and Thriller. The film includes many different elements which allow the film to fall under these three main genres. A drama is often described as a film â€Å"that relies on the emotional and relational development of realistic characters. † Most of the emotional development could be seen through the characters of Leonard and Natalie, who seem to develop an intimate relationship. This is in a way hinted to us during a scene where we see Leonard waking up in bed next to Natalie. Drama is also seen when â€Å"dramatic themes play a huge role in the plot. The purpose of a drama is to tell an honest story of human struggles; the struggles that Leonard goes through could be the same that one may go through when faced with the rare case of amnesia. The Mystery genre seems to be thrown at us with the many examples that we are given throughout the entire film. A mystery usually â€Å"centers on a person of authority, usually a detective that is trying to solve a mysterious crime. † The crime that is trying to be solved is the murder of Leonard’s wife, the murder is shrouded with mystery not only because of Leonard’s inability to remember, but also because of the very limited clues that we’re given. This genre could also tie into other genres such as Crime Fiction, Revenge, and Crime Thriller. The biggest element that the mystery genre has is the feeling of â€Å"whodunit†. This â€Å"whodunit† is usually described as the story that gives the identity of the murderer at the end. Memento also fits under the genre of Thriller which usually revolves around the anticipation and suspense in a film. They do this in their genres in order to keep the audience wanting more or for that edge of their seat excitement. Christopher Nolan does an excellent job with this genre; the way the film was presented helps this genre so much more. With the story being told to us backwards we want to know why things are happening, often times other films have us wondering what will happen next, but this film has us thinking, what happened before the event that we just saw. With the many genres presented, it’s no surprise that Memento would have an element of  confusion to it. The story itself may be confusing, but I rather want to talk about the characters within the film. Leonard Shelby suffers from a rare case of Anterograde Amnesia, according to the science section of the website HowStuffWorks. com anterograde amnesia means you have difficulty of making new memories and absorbing new information. Leonard Shelby being our main narrator throughout the film, we seem to trust and believe everything he tells us, this is due to the fact that we are so used to watching films that tell us that the narrator is a reliable subject. Leonard Shelby having this handicap makes us feel some sort of sympathy for him which makes believe he is reliable, but having a condition where you cannot remember how a conversation began once it has ended, leaves us wondering how can we believe this man if he can’t even remember how he got to the point in which he is now. We watch the film thinking of who is lying to us, who is telling us the truth, what’s going to happen, towards the end we seem to build a trust with our main narrator because we’ve seen the struggle he’s gone through and the situations  he’s had to face in order to find the murder who killed his wife. To our surprise, once we get reach the ending of the film, which in reality is the beginning, we see that Leonard has been lying to us the entire time. Character development in a film is crucial because it allows us to see who we can categorize to be the â€Å"good guy† or the â€Å"bad guy†, the non-linear narrative presented to us in Memento allowed Christopher Nolan to play with the way he presented his characters and make us believe the people who were truly lying to us.
